Student Forms
All forms need to be downloaded and completed and signed. Once completed, along with the required documentation, submit them to the Dean of Students, Dr. Frank Fudesco, at frankfudesco@northshorecollege.edu. If you’re unable to email Dr. Fudesco, you can submit your forms to any Student Services office. Dr. Fudesco can also be reached at 985-545-1239.
