Student Worker Checklist
Must be received 1 week prior to start date:
- Student Worker Application
- Copy of Eligibility Documents (See I-9 form)
- Social Security Card (a copy is required for payroll)
- Selective Service Card (required for all males 18-25) - www.sss.gov
- Personnel Action Request (PAR) - initiated by HR
- W-4 Federal Tax Withholding Form
- L-4 State Tax Withholding Form
- Primary Account Direct Deposit Enrollment Application & Voided Check for main bank account
- Secondary Account Direct Deposit Enrollment Application & Voided Check for secondary account (optional)
Employees must present the following the first day of work or will not be permitted to work:
- I-9 Employment Eligibility Verification Form
