NTCC Employee Application for Hurricane Ida Assistance

You have been invited to submit an application for a micro-grant, made possible by a generous donation from the Northshore Community Foundation.  Please complete the application below, including a brief explanation of how the funds will be used.

If you are chosen to receive a micro-grant, you may be asked to provide a photo of progress toward restoring your property and a brief statement about how the grant has helped you, so that we can demonstrate the impact of the micro-grant program.
